The Lump that didn’t hurt- But Needed Answers

It started as something easy to ignore a small, painless swelling near the cheek.

No fever. No discomfort. No loss of appetite.

For a 35-year-old male, this lump had been present for over a month, appearing shortly after a brief episode of fever. With no alarming symptoms, it could have easily been dismissed.

Tracing the Diagnosis

The swelling was located in the parotid region, an area where causes can range from simple inflammation to autoimmune conditions. Our in-house radiologist began with an ultrasound evaluation to explore possibilities such as inflammatory parotitis or even Sjögren’s syndrome.

Ultrasound, including color Doppler ultrasound, often serves as the first step in understanding soft tissue abnormalities, offering clarity without invasiveness. Yet, in this case, imaging raised questions that needed deeper answers.

To investigate further, a Fine Needle Aspiration Cytology- FNAC was performed.

What emerged under the microscope shifted the entire clinical perspective, Epithelioid granulomas.

Granulomas are not a diagnosis in themselves, they are clues. Often, they point toward chronic infections, with tuberculosis being one of the most significant considerations in the Indian clinical setting.

However, the Acid-Fast Bacilli AFB stain, commonly used to detect tuberculosis, came back negative.

And that’s where experience matters.

“In diagnostics, it’s important to read beyond isolated results. Patterns in cytology often guide us where individual tests may not.” mentions Dr. Smriti Singh, Consultant Pathologist at Star Imaging and Path Lab Limited 

Despite the negative stain, the cytological findings strongly suggested a tubercular origin. To confirm this, advanced molecular testing such as Genexpert (CBNAAT) was advised, tests that are far more sensitive in detecting hidden tubercular infections.

When Absence of Symptoms Misleads

What makes this case remarkable is not complexity, but subtlety.

No pain. No progression. No systemic signs.

Yet beneath that calm surface was a condition that required timely identification and targeted testing.
